Case Manager
This position is a full-time, 40 hours per week, salaried position. The Case Manager is responsible for providing comprehensive supportive services to all Charm City Care Connection participants. Services provided by the Case Manager include assessment of participant needs and goals, case planning, referrals, support with accessing benefits, ID documents, medical care, substance use treatment and providing warm handoffs to any other outside resources. Most importantly, this position builds long term partnerships with our participants in an effort to support them in reaching their goals. This position reports to the Manager of Supportive Services.
